A cranial hair prosthesis is a hair system designed specifically for those suffering from hair loss due to medical conditions or treatments, such as alopecia areata, alopecia totalis, trichotillomania, or chemotherapy. Unlike an off-the-shelf wig, each prosthesis is custom made. When a cranial prosthesis is needed due to medical-related hair loss, most insurance companies will cover from 80% to 100% of a cranial prosthesis.

How Do I Submit My Claim?

First. Make sure that your insurance covers reimbursement for Prescribed Cranial Prosthetics and will accept billing from providers that are not within their network.

Most insurance policies cover “Prescribed Cranial Prosthesis” but DO NOT cover wigs.

Insurance companies cover the cost under procedure codes S8095 and A9282

Get a letter and/or prescription from a licensed Oncologist or Dermatologist.

Make sure that the letter or prescription is for “Prescribed Cranial Prosthesis.” If your prescription is written using the term "wig" then your claim will most likely be refused.

Ellextensions will provide you with a receipt for your “Prescribed Cranial Prosthesis” showing proof of purchase.

Have the physician sign off on the insurance claim form and submit them together
