A cranial hair prosthesis is a hair system designed specifically for those suffering from hair loss due to medical conditions or treatments, such as alopecia areata, alopecia totalis, trichotillomania, or chemotherapy. Unlike an off-the-shelf wig, each prosthesis is custom made. When a cranial prosthesis is needed due to medical-related hair loss, most insurance companies will cover from 80% to 100% of a cranial prosthesis.

How Do I Submit My Claim?

• First. Make sure that your insurance covers reimbursement for Prescribed Cranial Prosthetics and will accept billing from providers that are not within their network. 

• Most insurance policies cover “Prescribed Cranial Prosthesis” but DO NOT cover wigs. 

• Insurance companies cover the cost under procedure codes S8095 and A9282

• Get a letter and/or prescription from a licensed Oncologist or Dermatologist.

• Make sure that the letter or prescription is for “Prescribed Cranial Prosthesis.” If your prescription is written using the term "wig" then your claim will most likely be refused.

• Ellextensions will provide you with a receipt for your “Prescribed Cranial Prosthesis” showing proof of purchase.

• Have the physician sign off on the insurance claim form and submit them together
